Making Smooth Transitions<\/h3>\n

After a hospital stay, it can be hard to adjust back to being home. Our Care Management team will make it easier. Care Managers can help you get appropriate follow-up care after an emergency room (ER) visit, hospitalization or significant life event, including medications or other medical supplies you need to continue your treatment. A care manager\u2019s support, as you transition between treatment settings, can help you better manage your health and reduce the need to go to the hospital.<\/p>\n
